Our Admissions Criteria

Low Acuity Behaviors Include:

•Mild to moderate resistance/combative with care

•Problematic verbal behaviors

•Physical restlessness/agitation

•Manipulative/Intrusive behaviors

•Obsessive-compulsive behavior; hoarding/stealing

•Poor intake/Excessive intake

•Elopement issues

•Inappropriate smoking habits

•Poor and unhealthy grooming/ hygiene habits

•Subjectively offensive behaviors.

• Is the person eligible for placement in an assisted living level of care?
  

• Does the person exhibit low acuity behavior problems that make it difficult for them to live in a more traditional care setting?
  

• Is the person diagnosed with or does the person have verifiable dementia, if considering the Dementia unit?
  

• Are the person's medical conditions and required treatments within the scope of Encanto Palm's caregiving capabilities?
  

• Does the person have no recent history of violent behavior against peers or caregiving staff?
  

• Does this person have no recent history of suicide or self-injurious behavior?
  

• Is payment authorized for admission?
  

Step 1Our clinical director reviews and approves the admission.

Step 2Encanto Palms completes an administrative review of the                  admission.

Step 3Approval from appropriate parties (HMOs, family members,

                  guardians/POAs, case managers, etc.) is obtained.

Step 4Appropriate admissions records are arranged.

Step 5        Transportation is arranged and verified for move in.
